Whether you are seeking services for yourself, referring a loved one, or submitting a referral as a provider, our team is here to walk beside you with compassion, dignity, and respect.
Once your referral is received, a Care Coordinator will reach out to review eligibility, explain services, gather information, and schedule an intake appointment.
Submitting a referral is the first step — it does not guarantee enrollment. Eligibility and service availability are determined through North Dakota Medicaid and program requirements.

- Adults living in North Dakota
- Individuals eligible or potentially eligible for Medicaid
- People with behavioral health needs who may benefit from 1915(i) services
