Whether you are seeking services for yourself, referring a loved one, or submitting a referral as a provider, our team is here to walk beside you with compassion, dignity, and respect.

Whether you are seeking services for yourself, referring a loved one, or submitting a referral as a provider, our team is here to walk beside you with compassion, dignity, and respect.

Once your referral is received, a Care Coordinator will reach out to review eligibility, explain services, gather information, and schedule an intake appointment.


Submitting a referral is the first step — it does not guarantee enrollment. Eligibility and service availability are determined through North Dakota Medicaid and program requirements.

Referrals are available to:


  • Adults living in North Dakota
  • Individuals eligible or potentially eligible for Medicaid
  • People with behavioral health needs who may benefit from 1915(i) services
     

If you are unsure about eligibility, submit the referral anyway — our team will help you review it.


Services through A New Path are currently available
