A New Path provides 1915(i) care coordination only. Your care coordinator works with you on person-centered planning, skill building, advocacy, and goal setting — and can connect you to peer support, non-medical transportation, housing, employment help, and other programs outside 1915(i). We come alongside you at your pace — with respect, dignity, and a belief in your God-given worth.
