You or someone on your behalf submits a referral or contacts us by phone or email. We welcome self-referrals, family referrals, and provider referrals.

A Care Coordinator reviews basic information, discusses Medicaid and 1915(i) eligibility, explains our services, and schedules an intake appointment.

Together, we identify your strengths, needs, and goals and create a person-centered support plan that reflects what matters most to you.

Your Care Coordinator and/or Peer Support Specialist stays connected, coordinates services, helps problem-solve barriers, and walks beside you as you work toward stability and recovery. 

Who Can Start?

Referrals are generally appropriate for adults who:


  • Live in North Dakota
  • Are eligible or potentially eligible for Medicaid
  • Have behavioral health needs that may qualify for 1915(i) services
  • Could benefit from support with stability, recovery, and community living


If you are unsure about eligibility, you are still welcome to reach out or submit a referral. Our team will help you review options and next steps.


 At this time, A New Path provides 1915(i) services in Cass and Ramsey counties only.

What This Process Is Not

 Submitting a referral does not guarantee enrollment. Service participation is determined by 1915(i) eligibility, assessment findings, and program availability.

A New Path follows North Dakota Medicaid guidelines and provides services that are client-directed, respectful, and aligned with each individual’s goals.
