A Care Coordinator reviews basic information, discusses Medicaid and 1915(i) eligibility, explains our services, and schedules an intake appointment.
Together, we identify your strengths, needs, and goals and create a person-centered support plan that reflects what matters most to you.
Your Care Coordinator and/or Peer Support Specialist stays connected, coordinates services, helps problem-solve barriers, and walks beside you as you work toward stability and recovery.
